Habilitation Staff

The following statements are intended to describe the general nature and level of work being performed and are not intended to be a complete list of all responsibilities, duties, or skills required.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Providing direct personal assistance with activities of daily living (grooming, eating, bathing, dressing, and personal hygiene);
  • Providing assistance with meal planning and preparation;
  • Securing and providing transportation (You must have a valid/current Texas Driver’s License enabling you to drive at all times.)
  • Providing assistance with housekeeping;
  • Providing assistance with ambulation and mobility;
  • Providing reinforcement of counseling and therapy activities;
  • Providing assistance with medications and the performance of tasks delegated by a Registered Nurse;
  • Providing supervision of individual’s safety and security;
  • Facilitating inclusion in community activities, use of natural supports, social interaction, participation in leisure activities, and development of socially valued behaviors; and
  • Providing habilitation, exclusive of day habilitation.
  • Adhere to state guidelines set forth by the Home and Community Based Services and Texas Home Living Handbook

Group Home Attendant

The direct care staff provides consumers with assistance and support necessary for the consumer to complete personal care, health maintenance, and independent living tasks; participate in community activities and develop, retain, and improve community living skills. The HCS Direct Care Staff reports to the Group Home Manager.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Provides continuous on-going active treatment by supporting all areas of Program training, operations, and the overall needs of the Consumer of the Group Home.
  • Provide transportation for residents,(You must have a valid/current Texas Driver’s License enabling you to drive at all times.)
  • Ensure the rights of the consumer, treating them with dignity and respect.
  • Complete all documentation required for consumers and Company operations.
  • Know and comply with all Company policies/guidelines per the policy and Employee Handbook/Guide books.
  • Conduct all matters in a professional and mature manner while representing the Company.
  • To stay alert and awake while on duty, ensuring a safe and clean living environment at all times.
  • Model and reinforce all needs of consumers and group home operations by “leading by example”.
  • Attend all in-services/training as scheduled to meet the needs of the Group Home, Consumer and the Company.
  • Communicate timely all consumer, home and vehicle needs to the Home Manager.
  • Assist/monitor all self-medication passes and the performance of tasks delegated by the Nurse. Immediately notify the nursing staff when an error is found or occurs.
  • Encourage and support consumer to be active in Community events, outings and opportunities. Facilitate social interaction and socially valued behavior. Give consumers choice of their activities.
  • Ensure menus and diets are followed.

Host home Companion Care (HHCC)

Must be 18 years or older. Must have a high school diploma, GED or complete a written competency-based assessment demonstrating the ability to provide HHCC and the ability to document the provision of host home/companion care and obtain 3 references stating you are able to do so.

Pass a back ground, Office of the attorney general, as well as National Employee Misconduct Registry check

Must have a valid Texas driver’s license.

Must be willing and able to assist with transportation to and from all appointments.

Must be able to provide support for individual’s needs

Paperwork required monthly before payment rendered

Preferred: Knowledge of individuals with intellectual and developmental disabilities, experience in Host home/companion care.

Foster/Companion Care contractor provides a supervised, 24-hour arrangement in a home for intellectual developmental disability individuals who are unable to continue living independently in their own homes.

They share a common living area.

The person receiving service pays provider for room and board.

Job Summary

Will assist in providing self-help skills training and therapeutic treatments to residents with intellectual and/or physical disability. 

Example Essential Job Functions:

  • Drive a company vehicle
  • Activities such as bathing, grooming, and dressing to train residents in daily self-care practices
  • Converse with residents to reinforce positive behaviors and to promote social interaction
  • Serve meals and eat with residents
  • Accompany residents on shopping trips
  • Restrain disruptive residents to prevent injury to themselves and others, using prescribed methods
  • Observe and document residents’ behavior, such as speech, feeding patterns
  • Document, report, and review residents’ daily progress, needs, medical issues, and behaviors
  • Transport or assist in transporting residents to and from medical appointments, activities, school
  • Work in other group homes, areas when needed
  • May work evenings, weekends, and holidays as needed.

Requirements:

High School Graduate or equivalent. Must be 18 years of age. Previous experience caring for children or adults with intellectual and/or physical disabilities preferred. Must be able to demonstrate competence in certain areas of training, i.e., CPR, CPI, First Aid, Behavior Management; Driving. Ability to recognize inappropriate or manipulative behavior in difficult or challenging situations. Ability to calmly and repetitively model appropriate behavior in difficult or challenging situations. Ability to react to situations, i.e., rescue, preventing a resident from hurting themselves. Valid Texas Drivers license, clear driving record.

Physical Requirements:

While performing the duties of this job, the employee is occasionally required to stand; sit and walk. Must be able to lift and/or move at least 75 pounds in order to assist in life skills, such as bathing, physical rescue, and wheel chair assistance. Exposure to outside weather conditions while working with residents. Noise level in the work environment is usually moderate.
